Description Dispensing chemotherapy for the inpatients. Reviewing chemotherapy prescriptions and ensuring the right dose, form, route and concentration is prescribed together with sign of the prescriber. Ensure the right raising of chemotherapy on the patient account. Consult with any prescriber, nurse or other health care professional. Supervise preparation, packaging, correct storage of drugs and check the procedures and product upon completion. Be responsible for all activities of pharmacy chemists to ensure that all such activities are performed completely, safely and without risk of harm to patients To ensure that any medication errors and/or near misses identified are reported via the Hospital error reporting system. To ensure all appropriate documentation records are made and maintained. To ensure correct and safe disposal of all unwanted medication and hazardous waste.
Qualifications have at least one year of pharmacy related work, have the ability to review the prescription of clinical doctor. experience in the same job in a clinical pharmacy.
